The application process begins by your completion and submission of a questionnaire and direction. The information is then reviewed to determine if the Osgoode Hall Innocence Project might be able to assist. For information on our screening process, please see our Mandate and Process page.

Please note that completion and submission of the questionnaire and direction does not ensure that your case will be accepted by the Osgoode Hall Innocence Project. Nor does the submission of those documents establish a lawyer/client relationship.

Please note further that although all information submitted will be treated as confidential by Osgoode Hall Innocence Project, we cannot guarantee the confidentiality of any e-mail transmissions from or via the web site.

Please follow the steps below to download and complete an application.

Application Process

Step 1: Before any action may be taken on your file, the Osgoode Hall Innocence Project must receive a paper/hardcopy of a signed Direction, allowing us to speak with any legal counsel that has represented you in the past or currently does so at this time.

To download a copy of the Direction, please click this link: Direction; save and print the Adobe Acrobat file and then please mail your completed direction to:

Osgoode Hall Innocence Project

Step 2: To complete the questionnaire, please right click on this link: Questionnaire, save and print the Adobe Acrobat file. This file requires Adobe Reader, which is free. Complete the questionnaire by hand and mail it to the address indicated on its last page.

Step 3: Please be patient. The application review process can be a lengthy one. Although we endeavour to be as expeditious as possible, we can offer no assurances as to when the review will be complete. It will almost certainly be several months.

If you are not able, or choose not to download files from this site, you can write or telephone the Innocence Project requesting a copy of the direction and questionnaire.  Follow this link to our Contact information.
